State inspection reports were obtained through Freedom of Information Act requests by The Midwest Center for Investigative Reporting filed with Illinois Department of Public Health, Indiana State Department of Health, Iowa Department of Public Health, The Texas Department of Housing and Community Affairs and Wisconsin Department of Workforce Development.

Database entries have been prepared by members of the Midwest Center staff, and are provided solely to aid in use of the reports. Midwest Center makes no representations or warranties as to the completeness or accuracy of the database, which was based upon judgments made from various differently formatted state records, many of which were handwritten or otherwise difficult to decipher or categorize.
